Every Prism van is a small clinic: exam space, supplies, a privacy curtain, and a team that knows the route. When the van parks, the awning comes out, the chairs unfold, and a neighborhood has a clinic for the day.

Heading to the next community

That's how care reaches more than 2,000 communities across Illinois: city blocks, suburbs, and rural counties where the nearest doctor can be an hour away. More than 1,000 healthcare workers (doctors, nurses, doulas, and community health workers) keep those routes running.

The vans have been at this a long time. Prism began as a community clinic on Devon Avenue in Chicago in 1986, and the idea never changed: if people can't get to care, care should get to them. We've been serving Illinois for forty years.
