Illinois's community health workforce, built here.
We train, credential, and launch community health workers and doulas, then stand behind them with curriculum, supervision, and a place on real care teams. It's the same model that built the state's only 24/7 hospital doula workforce, at Stroger Hospital with Cook County Health.
For community organizations, churches, health departments, and people who are already the ones their neighbors turn to.

Support for families navigating memory loss.
Memory loss changes life for the whole family. Prism supports families and caregivers navigating dementia, with plain-language education, caregiver support, and connection to community resources, brought to the places people already are.

Lead Poisoning Prevention
Lead testing for kids, right in the neighborhood.
Most lead comes from old paint in homes built before 1978, and most children with lead poisoning look and feel fine. The blood test is the only way to know. Prism brings blood lead testing for children to daycares, libraries, WIC sites, and churches across Chicago's South Side, with same-day results and counseling in English or Spanish.
