What happens at a clinic day

Our team will be onsite doing school and sports health exams, vaccines, and lead testing. We fill out the Illinois forms while you wait. Stop by and check if your child's records are up to date. Services are available while supplies last.

For the Whole Family

Flu and COVID shots, same visit.

Flu and COVID vaccines are available at these clinics for family members six months and older, provided with the Chicago Department of Public Health. Parents and guardians can get their flu or COVID shot during the same visit.

Cost and Billing

Care is open to everyone.

Families with insurance should bring their insurance card; covered services may be billed to their health plan with no surprise costs. Families without insurance pay based on what they can afford, and no one is turned away if they cannot pay.

With insurance

Most children with Medicaid or private insurance can get vaccines at no out-of-pocket cost. We confirm that with you before care.

If a letter comes later

If you are insured, you may receive a letter afterward called an Explanation of Benefits. This is not a bill.

No surprise bills

We never balance bill. You will not get a surprise bill from us.

Without insurance

You may qualify for our sliding fee scale, based on household size and income. We tell you about any cost before care. Funds and supplies for uninsured care are limited.

Our team speaks English and Spanish. Other languages are available on request at no cost.

Prism Health follows federal civil rights laws and does not treat people differently because of race, color, national origin, age, disability, or sex.

Your child's health information is protected by federal and state privacy laws, including HIPAA. The full notice is available at check-in. Privacy questions: compliance@prism.org.

Photos and video may be taken at events for Prism Health community updates. Children are not photographed without a signed parent or guardian release. Tell any team member if you prefer your family not appear.

Common Questions

What parents ask us.

Do I need to be at the visit?

No. You fill out the forms ahead of time and give consent. We call you the same day about anything that needs follow-up. If you want to come, you are welcome.

What will my child get?

On a lead-and-vaccine day: the blood lead test, the shots that are due, a growth check, the lead screening paper for daycare, and an updated shot record. We also help with insurance, food, and other programs. On a physical day, when a provider is with us, your child also gets the check-up, the vision and hearing screens, and the school health form.

Will my child get shots?

Only the shots that are due and that you did not decline on the Consent for Services. We give you a paper with each shot and check the state shot record.

What is the lead test?

A small blood draw from the arm. It is required at 12 and 24 months for every child in Illinois. Results take about 2 business days. We call you with the result. We do not put results in texts.

Does the school get everything?

The school gets the health form and shot record only if you said yes on the form. Nothing about mental health, drug or alcohol care, or sexual health goes to the school.

What if my teen wants to talk in private?

Illinois law lets teens 12 and older get some care on their own. Our team talks with your teen alone for part of the visit. This is normal. We tell you if there is a safety concern.

Can I get a ride? Is there a reward?

Every Illinois Medicaid plan gives free rides to visits: Aetna 1-866-329-4701 (2 business days ahead); Blue Cross Community Health Plan 1-877-831-3148 (3 days); CountyCare 312-864-8200 (3 days); Meridian 1-866-796-1165 (3 days); Molina 1-844-644-6354 (3 days).

Every plan also gives a reward when your child finishes shots or a check-up; ask your plan how to claim it. Illinois follows the American Academy of Pediatrics vaccine schedule; spacing shots out is not recommended.

What if my child has no insurance?

The visit still happens. Shots are free for children through age 18. We help you apply for All Kids and other programs the same day.

What if we get a letter from Medicaid?

Bring it or tell us. We help you update your address, report a pregnancy, start a renewal, or answer a work-rule letter. You have 30 days to answer some letters, so call us right away.

What does it cost?

When are school health records due?

Illinois school health records are due October 15. If they are late, your child may be kept out of class. Our team fills out the Illinois forms while you wait.

How do I reach you?

Call 1-800-325-1812, Monday to Friday, 9 am to 7 pm. An interpreter is free. For a mental health crisis, call or text 988. For an emergency, call 911.
